If you are having trouble filling out our forms using "FireFox" web browser, follow the instructions below.

Note about using "Firefox" web browser. If you cannot fill in the forms using Firefox, you must go to "TOOLS" in your browsers menu, typically located at the top of your browser window. Once you click on "tools" scroll down and select "OPTIONS" then select the "APPLICATIONS" tab, scroll down and find "PORTABLE DOCUMENT FORMAT" in the left column as you scroll down. To the right of that item it will have an action selected, click on it and a drop down menu will appear, make sure "USE ACROBAT (DEFAULT)" is selected. Firefox will not allow fillable forms unless that selection is made. Click "Ok" when finished.
